been meeting the most urgent needs of unhoused families in our
community. Our programs specialize in preventing family
homelessness when possible, providing shelter and transitional
housing to families in times of need, supporting the wellbeing and
academic achievement of children who experience homelessness, and
quickly returning families to safe and stable housing throughout

to thrive. What You'll Do We're excited to be adding a Case
Manager, Social Work to our growing team! This is an engaging and
rewarding opportunity that plays an integral role within the
organization. We are looking for someone who is passionate about
working with our participants and contributing to the fight against
homelessness. The Case Manager, Social Work will report directly to
the Director of Housing Services and be part of the Housing
Services Program team. The Housing Services Program helps prevent
families from becoming homeless and helps those experiencing
homelessness move as quickly as possible into permanent housing.
The program assists families to secure permanent housing through
housing search assistance, move-in financial assistance,
homelessness prevention, rent subsidies, and home-based case
management. What Your Day Looks Like Manage a caseload of
approximately 20 families, providing home-based and community-based
services across six countiesCollaborate with families to develop
and achieve goals related to housing stability, financial literacy,
employment, mental health, and moreAddress immediate family
challenges, such as domestic violence, housing crises, and
behavioral health issues, with appropriate interventions and
referralsAdvocate for families by building relationships with
service providers, landlords, and community organizations to
connect participants with resources and resolve barriersParticipate
in and organize family-focused events such as back-to-school
drives, holiday celebrations, and other community-building
initiativesAttend multidisciplinary case conferences and partner
with internal and external teams to ensure holistic support for
familiesMaintain accurate and timely records in case management
systems, documenting services, progress, and outcomes to meet
agency and compliance standardsWhat Makes You Successful in This

You Bring Master's degree in Social Work (MSW) from an accredited
institutionAt least 2 years of case management or social work
experience, particularly with low-income families, survivors of
domestic violence, or diverse populationsExperience providing
family-based interventions and using the five protective factors
for childrenUnderstanding of early child development needs and
resources for youthProficiency in case management platforms,
Microsoft Office Suite, and community resource navigation.
Experience with Salesforce is a plusFamiliarity with
trauma-informed care, housing-first principles, and harm reduction
approaches, and understanding of housing laws and family support
systemsStrong written and verbal communication skillsWillingness to
travel locally and long distances, work occasional evenings and
weekends, and adapt to diverse participant needsA background in
working with nonprofit organizations, social services
